A team of nurse practitioners who provide telehealth palliative care services to patients and families diagnosed with a chronic or terminal illness, focusing on symptom management to alleviate physical, emotional, or spiritual suffering and improve quality of life.

We know from personal experience how hard it is to navigate the healthcare system. We also know how hard it is for families to understand medications, coordinate care, and make decisions even with having a nurse in the family. We want to bring our advanced nursing education and coordination into every home affected by chronic disease. So, we teamed up to start a movement to change the community's perception and utilization of palliative care and increase access across the US one state at a time.

Palliative care is a specialty service that relieves distressing symptoms, addresses emotional and psychological needs, and collaborates with other healthcare providers to seamlessly coordinate medical treatments, therapies, and support services. This results in more comprehensive and holistic care for patients and families.
